“The Patient Looks a Little Codey” #warning

Source: @nursemikael_

Nurse concern—particularly from experienced nurses—is increasingly recognized as an independent predictor of patient deterioration, even when vital signs or observations (obs) appear within normal limits. Seasoned nurses develop a deep, intuitive understanding of what a stable patient looks like, often picking up on subtle changes in behavior, appearance, or communication that machines and charts may miss. This gut feeling, sometimes described as “clinical intuition,” can be an early warning sign that something is going wrong before it becomes measurable.

Studies have shown that when a nurse expresses concern about a patient’s condition, it should be taken seriously—even if the numbers don’t yet reflect a problem. Their insight is shaped by years of hands-on experience and repeated exposure to early warning signs of deterioration. That’s why many hospitals are now incorporating nurse concern into escalation protocols and rapid response systems. It’s a powerful reminder that human judgment, especially from trained professionals, is a critical part of safe and effective care.
